Purpose of the Notice of Privacy Practices

This Notice of Privacy Practices is meant to inform you of the uses and disclosures of protected health information that we may make. It also describes your rights to access and control your protected health information and certain obligations we have regarding the use and disclosure of your protected health information.

Information regarding your health care, including payment for health care, is protected by Connecticut Law Federal Law including, the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), 45 CFR parts 160 & 164 and the federal law regarding Confidentiality of Alcohol and Drug Abuse Patient Records, 42 CFR part 2.

How We May Use or Disclose Your Protected Health Information

Your protected health information is needed for treatment, payment and health care operations and will only be used with your written approval.
